Terms and Conditions for 4 Day Cruise Plus

Fully Paid Cruise including Accommodations

1.

To validate this offer, complete this registration form, and mail with $287.70 USD within 21 days from date of issue, or this offer will be void. The $287.70 is comprised of all applicable port charges, government taxes and surcharges in the amount of $131.35 per person (subject to change), and a $25.00 hotel tax. You must also provide us with proof of address, i.e., copy of phone bill or electric bill, and proof of citizenship, i.e., copy of your passport or birth certificate.

You will receive an acknowledgment within 14 days of the receipt of your registration form and all required fees. You will be contacted at least 45 days prior to your requested departure date with the status of your reservation.

Select 2 valid dates at least 21 days apart, in chronological order. These dates must be for departure on Monday’s from Los Angeles, California, or Sunday’s from Port Canaveral, Florida. Holiday Travel of America (HTOA) must receive your dates at least 90 days in advance of the earliest departure date, with all applicable port charges and government taxes. Reservations are space available.

Passengers traveling on this offer must be at least 25 years of age and possess 2 major credit cards. This offer is non-transferable and identification will be required.

Your certificate is valid for your hotel guestroom (three (3) days/two (2) nights in Anaheim, California or four (4) days/three (3) nights in Orlando, Florida) and passage for two adults to cruise. Additional room nights are available, at discounted rates. Miscellaneous expenses, gratuities, service charges, alcoholic beverages and hotel food are not included in this offer. Airline tickets and/or airport transfers to/from your home/hotel/port of departure are not included, but can be purchased from HTOA.

Staterooms are Category 1 Upper/Lower (bunk beds). Reservations are accepted on an individual basis only. Two or more request forms received together requesting the same travel dates will not be accepted.

7.

All reservations are subject to availability, and are restricted, in respect to the specified cruises, dates of travel and stateroom category. No travel will be permitted during peak and holiday periods (this includes one week prior to, and one week following holidays). This promotion is subject to high season blocked out periods. All cruises are subject to space availability. The following is a list of blocked out periods for these cruises: December 15-January 17, annually; June 7 - August 28, annually; and holidays: New Year's Day, Martin Luther King’s Birthday, President’s Day, Easter Week, Memorial Day, Independence Day, Labor Day, Columbus Day, Thanksgiving Day, and Christmas.

Any confirmed reservation that is cancelled will incur cancellation fees from $50.00-$75.00, plus all related port charges, taxes, surcharges and the certificate will be void.

This cruise offer cannot be sold, bartered, exchanged, or transferred to another party.

10.

Third and fourth person upgrade, when available, must be purchased through Holiday Travel of America at cruise lines current published brochure rates.

All reservation requests will be processed by mail only, in a timely manner. If you require additional information, please write to the address shown on the front of this offer.

This offer is not valid for groups, and cannot be used in conjunction with ay other cruise offer. This program is non-transferable, cannot be sold to the end user, and cannot be redeemed for cash. It is important that you do not leave for your cruise until you have received your confirmation number for your cabin. Cruise documents will be available at the pier on the day of sailing. This offer is for the cruise and room nights only, airfare allowances do not apply, and cannot be used in conjunction with this award.

This offer is void where prohibited by law. No other representations, oral or otherwise, are valid in conjunction with this offer. Offer subject to change without notice. For promotional use only. Not for sale. Due to the promotional nature of this offer, HTOA can only honor one certificate, per household, per year.

HTOA is not to be held responsible for any act of God, and/or any other circumstances beyond our control, causing cruises and accommodations to become unavailable. In the event that such a circumstance occurs, Holiday Travel of America will offer a substitute cruise and/or accommodation destination.

Both passengers must sign the form below or this certificate is void.

This certificate expires one year from date of issue.

No facsimiles or photocopies of this certificate will be accepted; original documents only.

HTOA is not responsible for any verbal or written representation, by any person or company, which is not contained in this certificate.
