1. Please complete the Gift Card Claim Voucher attached and send it in an envelope by certified mail only, which must be postmarked within 21 days from the sponsors issue date. This offer is NON-TRANSFERRABLE and you must provide a photocopy of a government issued photo identification to confirm the promotional offer was not transferred to a third party. Upon validation of the form, HTOA, Inc. will then mail you a Gift Card Request Form, including the terms and conditions of this offer.
