To validate, complete the following reservation request form and mail within 21 days from date of issue or this certificate will be void. Holiday Travel of America (HTOA) will send an acknowledgement letter, requesting an estimated airline and hotel tax of $79.00USD and a refundable reservation deposit of $50.00USD. You are responsible for all applicable hotel and airline taxes, and must return your tax deposit within 21 days of receipt of that letter. No facsimiles or photo copies will be accepted; original documents only. Please keep a copy for your record. This offer expires 1 year from date of issue.

When you receive your acknowledgement letter, select three valid dates at least 21 days between choices, and in chronological order. These dates must be for arrival on Tuesday only. Upgrades to weekends may be available. HTOA must receive these dates at least 60 days in advance of earliest departure date. You will be contacted 45 days prior to your confirmed travel date. This promotion is subject to high season blocked out periods. It is not valid during major holiday seasons (this includes one week prior to and one week following). Major holidays include: New Year’s Day, Martin Luther King’s Birthday, President’s Day, Easter, Memorial Day, Independence Day, Labor Day, Columbus Day, Thanksgiving Day, and Christmas. All charges incurred such as taxes, gratuities, and incidental expenses are the responsibility of the traveler. This offer is not valid for groups, and cannot be used in conjunction with any other vacation offer. One certificate per household, per family, per year may be redeemed. This certificate has no cash redemption value. Cancellations must be made in writing, and penalties may apply once reservations have been confirmed. This package is subject to change without notice.

Both passengers traveling must be 21 years of age or older. This offer is invalid if sold, bartered, exchanged or transferred to another party. This offer is void where prohibited by law.

Accommodations for this promotion are standard hotel rooms. All hotels are subject to change and are based on space availability.
